Why Kardio Remote Patient Monitoring?

What is Kardio Remote Patient Monitoring?
Kardio Remote Patient Monitoring is the ability to remotely keep track of patients health between doctor visits by utilizing IoT-enabled smart technology. This includes monitoring patient’s blood pressure, weight, blood glucose levels, heart rate and other measurements and then passing this information along to a CHP (Certified Healthcare Professional) in an easy to access way.
Kardio provides patients all the tools to successfully enroll in Kardio Remote Patient Monitoring including free FDA-approved devices, customer setup, training and support, and a state of the art of remote patient monitoring. Kardio Remote Patient Monitoring is a convenient system designed to allow you to monitor multiple patients efficiently between visits so you can have better control of their chronic conditions. What do I need to do?
You only need to put devices where it needs to be and it automatically send the measured data to your caregiver’s web portal and/or open your APP daily and follow the instructions prescribed.
Who can see my data?
Your care team will be monitoring your data periodically. Your data is saved in a secured, HIPAA compliant health cloud hosted in the USA.
What happens to my data?
Your data is sent to your care team to verify periodically how your health condition is progressing. Your care team will also receive alerts as soon as your vitals stray from pre-determined ranges set by your physician and will contact you to adjust and intervene as required.
How do I sign up?
Talk to your physician today to learn more and enroll in the program. A Kardio staff member will set you up on the program, help you download the APP and walk you through the entire process of taking your vitals. We are just a phone call away if you need any further help.
Can I opt out of the program?
Of course, you can, after initial mutual agreement and/or per by doctor’s prescription but you will lose benefits of the program such as easy management of your health condition, reducing hospital visits and better access to your care team. If you’re unhappy about your program, talk to your care team about your concerns and we’ll try to address them the best we can.
How long do I need to participate in the program?
It’s usually requires minimum of 12 months (or 1 year initial term) and it’s up to you and your doctor to decide what’s best for your health and how long you need to participate in the program once after initial agreement.
Am I liable if the equipment is lost or damaged?
If you have lost or damaged equipment, please contact Kardio care team to arrange for a replacement if happens within Kardio warranty period of 1 year. All equipment belongs to Kardio and must return all equipment once the agreement is expired and must return to Kardio within 15 days of termination. Otherwise, patient will be liable for the payment of each and every equipment used during the remote patient monitoring management with Kardio.
What kind of support can I expect if there are issues with the APP or Devices?

What benefits can I expect from participanting?
Actively participating in an RPM program may improve your quality of life by proactively managing your health condition, reducing hospital visits and minimizing personal costs such as travel and wait time, while giving you better access to your care team.
Through secure 2-way HIPAA-compliant communications, you can reach your care team through text, photos or video. Please note that 2-way secure messaging does not replace urgent care. If you need urgent care, please call 9-1-1.
How does it work?
It all starts with an app that you download on your personal iPhone or Android device. Depending on your program, you may be provided with the following FDA-approved 4G or wireless devices: blood pressure monitor, body scale, glucometer, pulse oximeter or spirometer. All devices are connecting through either 4G data plan or Bluetooth to your phone sharing your vitals with your care team through an encrypted HIPAA-compliant wireless transmission. All devices are provided at no cost to you for your personal use and compliance in the program but must return to Kardio once the program ends and/or agreement terminates after initial term of 1 year.
